Personalised Wellbeing Plans

A few days ago Dementia United highlighted the latest resource from Dementia Change Action Network #DCAN to support  the experience of emergency hospitalisation for people living with dementia and their carers, as well as supporting emergency hospital staff.

The Personal Wellbeing Plan for Emergencies is a document to facilitate personalised care and support in busy hospital departments.

It would be great if you could help encourage its use by people living with dementia, carers, and people who work in emergency hospital environments, and share the attachments plus this message with any networks and colleagues who you think might find this of interest.

A few ideas from DCAN to support sharing this resource:

  • Share the launch tweet
  • Share their launch post on LinkedIn
  • Add the resource to your next company newsletter (copy suggested below)
  • Use the attached assets to create your own social media posts
  • Forward this email onto an individual or organisation who might be interested in this work

Dementia Change Action Network (DCAN) has been exploring dementia care in emergency hospital settings, looking at ways to make it better for everyone.

In collaboration with people with lived experience and the NHSE they have co-designed a new document which is for people to take to hospital with them to facilitate the delivery of personalised care and support in busy hospital departments.
