The Modern Service Framework (MSF) for Dementia and Frailty is a national initiative announced under the government’s 10-year health plan to standardise care, reduce regional variation, and improve diagnosis, treatment, and support across health and social care. The framework is scheduled for publication by the end of 2026.
The ambition of the MSF is that in 10 years’ time, anyone living with frailty or dementia and their unpaid carers will live well in their neighbourhood – through timely diagnosis and high quality, proactive, joined up health and social care.
Priority areas include:
- Timely identification and diagnosis of frailty and / or dementia
- Effective proactive, planned, and responsive care and support
- Better support for unpaid carers
- Joined up care across all services
There is currently a national task and finish group working on this, but they are seeking comments and feedback which can inform their work.
We know that so many of our providers daily support GM residents who are frail or living with dementia.
Whilst these may be clinical diagnoses, the whole system of health and care are needed to provide support and care.
How can we strengthen the role of social care within these priorities?
What are the barriers and critical enablers?
We would like to strongly encourage our GM providers, who have experience and knowledge of supporting people with frailty and / or dementia to direct any comments to the MSF frailty and dementia inbox – england.fdmsf@nhs.net
