The GIRFT Urgent and Emergency Care (UEC) team and the Nursing Quality Team at NHS England, are looking to highlight innovative practice that helps avoid inappropriate hospital admissions for people living in care, residential or nursing homes.
We want to develop a small series of case studies showcasing learning and innovation in this area. If you, or your networks, are aware of examples of great practice that should be shared more widely across the healthcare community, we’d love to hear from you.
